2) The name and address of any Guardian Ad Litem appointed for the minor;
3) The names and addresses of the natural guardians or other persons having legal custody of the minor;
4) The consent of the natural guardians or other persons having legal custody and authority to act upon behalf of the minor child;
5) A statement disclosing the interest of any natural or court-appointed guardian whose interest may be in conflict with that of the minor;
6) A description of the cause of action in which the minor’s interests arises;
7) A summary of the terms of the proposed settlement;
8) The proposed distribution of the net proceeds in conformity with Florida Law;
9) Copies of all agreements, releases, closing statement or other documents to be executed on behalf of the minor;
10) The report of the Guardian Ad Litem, if any;
11) A statement stating whether it is believed to be in the minor’s best interest for this settlement to be entered;
